Entering is easy – just fill in the form and send it to us In order to enter all you need to do is download the entry form or forms in the categories you would like to enter. Each entry form has a series of questions relevant to the category in question and all you need to do is complete that the form or forms and email or post them to us. Entry forms can be emailed to cathy.heys@cch.co.uk with the subject heading the Pay Awards 2008 or mailed to Cathy Heys, Pay Awards, CCH, 145 London Road, Kingston, Surrey, KT2 6SR.
